Guidelines and Forms

Thyrogen is covered by Medicare, Medicaid and most private insurance plans. Depending on the individual plan, Thyrogen is sometimes covered as a medical benefit and sometimes as a pharmacy benefit. The ThyrogenONE program will assist you and your patients with understanding the specific requirements of your patients' benefits.

Occasionally, additional documentation is needed in order to obtain prior authorization for reimbursement of Thyrogen. Once you have registered with the program, a ThyrogenONE specialist will provide the appropriate forms and contact information.

Statement of Medical Necessity
Some insurers may require additional information to justify the necessity of Thyrogen. This form is designed to give details on why Thyrogen should be administered.

Safety Information
Thyrogen® (thyrotropin alfa for injection) is indicated for use as an adjunctive diagnostic tool for serum thyroglobulin (Tg) testing with or without radioiodine whole-body scan (WBS) in the follow-up of patients with well-differentiated thyroid cancer. It is a prescription product injected into the muscle. Thyrogen® may not be appropriate for all patients. The most commonly reported adverse events are headache, nausea, weakness and vomiting. Even with a Thyrogen®-stimulated Tg testing and WBS, a risk remains of missing a diagnosis of thyroid cancer or of underestimating the extent of disease. Adverse events should be reported promptly to Genzyme Medical Information at 1-800-745-4447.
