Getting Started with ThyrogenONE

Genzyme is committed to supporting its Thyrogen customers. We are very excited about the ThyrogenONE program and the high quality of services that it will provide.

Registration

Enrollment is simple:

Complete the one page Prescriber Registration Form (PDF).

Fax the completed form to the ThyrogenONE service center at 888-326-1002.

You will receive a return fax acknowledging receipt of your registration and providing you with a ThyrogenONE (T-1) identification number. Use the T-1 number in all of your correspondence with the ThyrogenONE service center to receive timesaving, personalized support.

Requesting Verification of Benefits

Once you are registered, simply follow these steps to request timely verification of benefits for all of your Thyrogen patients:

Obtain a signed Authorization for Release of Health Information (PDF) from your patient.

Complete a T-1 Referral Form (PDF).
Please be sure to initial the Consent disclaimer at the bottom of the form.

Fax the completed T-1 form to ThyrogenONE at 1-888-326-1002.

A reimbursement specialist from a Thyrogen specialty distributor will contact you to report the results of the benefits investigation and to answer any questions you may have.You will also receive a fax from ThyrogenONE reporting the results of the benefits investigation.


Safety Information
Thyrogen® (thyrotropin alfa for injection) is indicated for use as an adjunctive diagnostic tool for serum thyroglobulin (Tg) testing with or without radioiodine whole-body scan (WBS) in the follow-up of patients with well-differentiated thyroid cancer. It is a prescription product injected into the muscle. Thyrogen® may not be appropriate for all patients. The most commonly reported adverse events are headache, nausea, weakness and vomiting. Even with a Thyrogen®-stimulated Tg testing and WBS, a risk remains of missing a diagnosis of thyroid cancer or of underestimating the extent of disease. Adverse events should be reported promptly to Genzyme Medical Information at 1-800-745-4447.
